need help solving => ICD3Err0048: Unable to set trace options

My code compiles fine on ICD3 with MPLAB v8.92, and programs fine, but the following error pops up and blocks all debugging functions:
 
ICD3Err0048: Unable to set trace options
 
I have JTAG disabled in the CPU config settings, so I don't think JTAG is the problem.
 
I cannot find the error "ICD3Err0048" anywhere online, so hoping someone here can offer a few suggestions of what to try.

    Is the ICD plugged directly into your PC, or via a hub?
    Is it a USB2 port, or a USB3 one?
    (USB3 ports are usually labelled "SS", or have a blue body)

    - Please use latest firmware for ICD3
    - This is my experience(unscientific method)...
    I am getting various issues when use ICD3 with MPLAB X.
    JTAG option or cable connections are not problem in this case.
    At that time, I program PIC32 with project codes.
    Then, try to debug again.
     
    If still get issue, try to program and debug again.
    Of course, there are many forums with ICD3 issues.
    But, they are not solving unknown issues.

    Thanks for the quick reply. ICD3 is directly plugged to PC (no hub) on a USB 2 port. Which is better?
    I think there may be a debugger memory region conflict with a bootloader section.
